As a practice that specializes in Integrative & Functional Medicine Nutrition, we take up to 2 hours with each of our new patients to establish a partnership and formulate a medical nutrition plan that is both safe and effective using the functional medicine model. It is an individualized, patient-centered, science-based approach that empowers patients and practitioners to work together to address the underlying causes of disease and promote optimal wellness.

 

We will complete a detailed review of your nutrition, biochemical, lifestyle and genetic factors and leverage that data to create a personalized treatment plan that leads to improved outcomes. By addressing root cause, rather than symptoms, we can reduce disease risk and help you reach your wellness goals. Sometimes one condition has many different causes and, likewise, one cause may result in many different conditions. As a result, every care plan is unique and established in visit case by case. 

 

In addition to review of your past nutrition and medical history, we will perform an in-depth assessment of your anthropometrics (weight, ht, etc), laboratory values, symptoms and conditions that can be treated with traditional and functional medical nutrition therapy. We will determine the best nutrition approach based on our discussion and create a custom plan for you that may include recommendations for testing or collaboration with other health professionals to help identify and treat root cause (antecedents, triggering events, and mediators/perpetuators).

 

At the end of each visit we will establish short and long-term goals, and decide how often you would like to meet for accountability and continuity of care. Follow-ups may vary from 30-120 minutes depending on the complexity of the case.
