2. Complete New Patient Paperwork

Once you have activated your invitation to the portal, you will be prompted to complete our New Patient Forms within the system. If you opt out of the portal, let your dietitian know at time of scheduling, and we will mail them to you.

Completing our forms is essential to helping your Dietitian develop a nutrition/wellness program that addresses your needs, goals, and interests, and that is safe and effective. They include medical release and HIPAA forms that allow us to communicate with your health care providers by sending recommendations and receiving test results we have requested on your behalf. 


This information is also needed for us to correctly submit claims to your insurance company for coverage. Although visits are free or of little cost to you with most insurance plans we accept, we encourage you to check with your insurance company prior to your appointment.


Our state of the art electronic medical record is where all your medical information is securely kept. The portal is the fastest way for you to communicate with your dietitian between visits.

Your portal allows you to send secure messages and attachments, access your health information and care plans, request and reschedule appointments, submit payment information securely, and utilize the free health tools available to you as a patient of our practice.

3. Download Our Patient Portal App (Recommended)

We strongly advise mobile users to download our wonderful patient portal app called "Healthie", where the portal's valuable tools and resources are easily accessed. TIP: In order to receive notifications when you receive messages from your dietitian, be sure to accept push notifications.


Helpful Information:

  • You will receive text and email appointment reminders from our office prior to your appointment.

  • We ask that you be on time, and reserve the right to cancel an appointment if arrival is later than 15 minutes. This is in observation of other patient appointment times.

  • As with office appointments, Telehealth appointments may start 5-10 minutes from your appointment time. We appreciate your patience as we care for other patients.  

  • We request 24 hours notice of cancellation, and charge a fee of $80 for missed appointments and late cancellations as we cannot double book.

  • Please bring your insurance card and a form of photo identification to your first appointment. 

  • Payment is requested at time of service for non-covered visits, testing fees, and copays/coinsurance. We gladly accept cash, checks, and credit card.
