At your new patient appointment, we break the ice and discuss what you and your doctor would like us to help you or your loved one achieve, in addition to discussing and determining existing nutritional needs.
We will discuss your past nutrition and medical history, including an in depth assessment of your anthropometrics (weight, ht, etc), laboratory values, and symptoms that can be treated, cured, or prevented with medical nutrition therapy. We will determine the best therapy for treatment based on our discussion and create a nutrition plan for you.
We will begin to establish short and long-term goals, and decide how often you would like to meet for visits for accountability and/or continued care.
For limited time appointments, such as private pay appointments, it is particularly important that we receive your New Patient Intake Form prior to your appointment. You can download the paper form from the Our Practice tab, or fill out inside your Patient Portal. This information is essential to helping your Dietitian develop a nutrition/wellness program that addresses your needs, goals, and interests, and that is safe and effective.
